What is Integrated Care?

Integrated healthcare is the systematic coordination of general and mental healthcare. This model focuses on whole-person care to address each patient’s mental health needs in collaboration with their primary care provider. Integrating mental health and primary care services improves access and quality of care for people with multiple healthcare needs.

Services include:

  • Diagnoistic assessments
  • Medication management
  • Psychotherapy
  • Care coordination
  • Linkage to services

Meet Our Team!

Kimberly Mapstone, LPC

Kim has been working in mental healthcare for just under 30 years and is still passionate about her work. She received her Master’s in Human Services from Lincoln University which has provided her with the opportunity to integrate her valuable life experience with psychological and social therapy in the counseling setting. She is a warm and caring counselor who works with all mental health issues to help others achieve inner healing through connecting with their feelings and their true selves.  She provides an eclectic approach in order to meet others where they are and grow from there. She specializes in trauma, depression, anxiety, grief, LGBTQ+, drug and alcohol, and teenagers.  Kim spends time hanging out with friends, walking her four dogs, dancing, kayaking and especially enjoys her wife and other family member.

Monique Smith, Program Coordinator

Monique Smith is the Program Coordinator for the Integrated Care Services team. She will be the one coordinating services, setting up patients with resources, referrals, registration/intake, and scheduling appointments to see Stephanie, LCSW and/or Dr. T, the psychiatrist. If you are a Metro patient and are interested in ICS, please stop in and see Monique! During her personal time, Monique enjoys spending time with her family, traveling, and learning about diversity/culture of others.
